Minor Boot Camp Participation Liability Waiver & Release
Location: 620 13th St, Highland, IL

This Liability Waiver and Release of Claims (“Agreement”) is made between AlphAs Gym LLC (“AlphAs”) and the undersigned parent or legal guardian (“Parent/Guardian”) on behalf of the minor child identified below (“Participant”).

By signing this Agreement, the Parent/Guardian agrees to the following:

1. Description of Program

I understand that my child will be participating in a youth boot camp–style fitness program designed for children approximately 8–12 years old.
Activities may include, but are not limited to:

Bodyweight exercises

Running, jumping, and agility drills

Fitness games

Light resistance or conditioning exercises

Age-appropriate strength and cardio movements

I understand these activities are physically active in nature.

2. Assumption of Risk

I acknowledge that physical activity involves inherent risks, including but not limited to:

Muscle soreness, strains, or sprains

Slips, trips, or falls

Accidental contact with equipment or other participants

Minor injuries associated with physical exertion

I knowingly and voluntarily assume all risks, both known and unknown, associated with my child’s participation.

3. Medical Fitness & Disclosure

I certify that my child is physically able to participate in this program and does not have any medical condition that would prevent safe participation unless disclosed in writing to AlphAs Gym LLC prior to participation.

I agree to inform AlphAs staff of any:

Medical conditions

Injuries or limitations

Allergies

Special needs

4. Supervision Acknowledgment

I understand that AlphAs staff will instruct and supervise group activities, but cannot provide constant one-on-one supervision.
I acknowledge that my child is expected to follow instructions, rules, and safety guidelines at all times.

5. Release of Liability

In consideration for my child being allowed to participate, I release, waive, and hold harmless AlphAs Gym LLC, its owners, employees, coaches, agents, and representatives from any and all claims, demands, or causes of action arising out of my child’s participation, including injury or property damage, to the fullest extent permitted by Illinois law, including claims based on negligence.

6. Rules, Conduct & Removal

I understand that:

Safety rules must be followed at all times

Unsafe or disruptive behavior is not permitted

AlphAs reserves the right to remove a participant from the program for safety or conduct reasons

7. Emergency Medical Authorization

In the event of an emergency, I authorize AlphAs Gym LLC and its representatives to obtain medical treatment for my child.
I agree to be financially responsible for any medical costs incurred.

8. Photo & Video Release

I grant permission for AlphAs Gym LLC to use photographs or video recordings of my child taken during the program for promotional or educational purposes, including social media and marketing.
If I do not wish to grant this permission, I will notify AlphAs Gym LLC in writing prior to participation.

9. Governing Law

This Agreement shall be governed by and interpreted according to the laws of the State of Illinois.

10. Acknowledgment & Signature

I certify that I am the legal parent or guardian of the child listed below.
I have read and understand this Agreement and sign it voluntarily.
